For your first visit, we will need certain information on your current health to inform our dentists and other specific staff. Before you arrive, please download the forms below that apply to your situation, print them and fill them out. Then when you arrive, please give the completed forms to the staff member at the front desk. This will prevent delays in your being seen by our dentist or hygienist.
